Community Team Leader

About the Role
  • Seeking an experienced registered nurse,with background in home care to fulfil the role of Community Team Leader, based in the Kimberley Town of Kununurra.

    This position will provide you with positive challenges in coordinating, leading and delivering CHSP, Home Care Packages and centre based activities in the Kununurra and surrounding areas including providing direct clinical care to clients.You will ensure all services are provided in a manner that recognises the importance of helping clients to maintain their quality of life and model the core values and guiding principles of Juniper.

    The role will require you to: - Provide comprehensive clinical assessment and review of clients' care needs - Provide direct clinical care to clients - Develop care plans and coordinating the delivery of CHSP and home care packages - Provide leadership and training to Community Support Workers - Actively support and working cohesively with the wider care team - Facilitate and coordinate the access process for new community clients
Experience & Skills
  • APHRA registration as a RN
  • Experience leading clinical and/or care teams (desirable)
  • Comprehensive knowledge of evidence based clinical and care practices Demonstrated knowledge of continuous improvement, accreditation requirements, and documentation
  • Experience with clinical problem solving and a commitment to delivering high quality, consumer directed care
  • Familiarity with Medicare and DEX reporting
  • Knowledge of Procura (desirable)

  • This is a permanent full time position working Monday to Friday
  • Hourly rate of between $42.56- $44.94 per hour
  • Salary packaging of up to $15, 900 increasing your take home pay
  • Travel/ relocation assistance of up to $750.

The Kimberley is located in the north-west region of Western Australia. It covers an area of 421,451 square kilometres, an area three times the size of England, but is home to only 35,748 people. A relatively high proportion of the residents are Aboriginal Australians. The population swells seasonally with an influx of tourists and a mobile working population.

The major towns of the Kimberley include Broome and Derby on the west coast, with Kununurra and Wyndham in the east. Within this vast landscape there is a huge diversity of scenery to be explored and adventures to be experienced. Broome enjoys a laid-back tropical ambience whilst the ranges of the East Kimberley showcase the awe-inspiring grandeur and beauty that nature has created. Venturing into this land imparts a unique sense of connection with an ancient and unspoiled part of Australia. Travels across the Kimberley will yield tropical beaches, rugged ranges, spectacular gorges, waterfalls, caves, pockets of lush rainforest and an astounding variety of wildlife.

The region has a thriving economy, based on both tourism and the richness of its natural resources. It enjoys regular commercial flights from Perth and interstate capital cities. The Kimberley has a tropical summer and dry winter and this diverse climate changes the landscape from season to season.

There are approximately 70 general practitioners including GP registrars in the region, nine in private practice, 33 in Aboriginal Medical Services, two in the Royal Flying Doctor Service and 26 hospital based general practitioners.
