Outreach Services

Medical Outreach Indigenous Chronic Disease Program

Medical Outreach Indigenous Chronic Disease Program (MOICDP) is funded by the Australian Government Department of Health and administered by Rural Health West.

This program aims to increase access to a range of health services, including expanded primary health care for Indigenous people. By increasing access to multidisciplinary care, MOICDP aims to broaden the range or services offered by visiting health professionals to prevent, detect and manage chronic disease more effectively.

MOICDP focuses on a multidisciplinary team approach to health care with a focus on five chronic conditions:

  • Diabetes
  • Cardiovascular disease
  • Chronic respiratory disease
  • Chronic renal (kidney) disease
  • Cancer
For further information on MOICDP, download the MOICDP Service Delivery Standards.

Eligible locations for MOICDP services
The Australian Government Department of Health uses the Australian Standard Geographical Classification Remoteness Areas (ASGC-RA) system to determine eligibility for service locations across Australia. Services delivered to Aboriginal and Torres Strait Islander communities, supported through MOICDP, range from RA 1 (major cities) to RA 5 (very remote).

The Service Delivery Standards require that all service providers funded under MOICDP demonstrate that they have undertaken appropriate cultural awareness and safety training, or be willing to undertake training prior to commencing service delivery. For further information, please go to Cultural Awareness and Safety Training.

Reporting requirements
Service Providers are obliged to submit a Service Activity Report and Tax Invoice within two months of the completion of an approved outreach service. To download the Service Activity Report please click on the link below:
