Chronic Disease Nurse / Diabetic Educator

About the Role The role will include planning, coordination, delivery, assessment, advocacy and support to the client group. The Chronic Disease Nurse / Diabetic Educator will work as part of a team in a multi-disciplinary to trans-disciplinary model, sharing generic work and offering discipline specific interventions. The Chonic Disease Nurse / Diabetic Educator will: -Participate in Quality Improvement Activities. -Provide culturally appropriate assessment, therapeutic and education services for clients with chronic disease as required. -Ensure that clients receive education on the use of therapies and treatments prescribed and feedback on the outcomes of therapy and treatment. - Adhere to relevant professional Code of Ethics guidelines as well as PHN Code of Conduct Guidelines. - Participate in clinical and peer meetings. - Record progress notes and other contacts with clients according to the policies and procedures of the Pilbara Health Network - Proactively develop and maintain formal networks with other services and organisations who have a role in the support of the wellbeing of individuals living in the Pilbara and develop shared care plans as needed.
Experience & Skills General Nursing degree, qualifications in Diabetic Education and membership of AHPRA. -Demonstrated experience in working in a Primary Health care setting and working with chronic disease patients - Demonstrated experience in working in a role of Diabetic Education - Strong commitment to the advancement of Aboriginal issues and service equity - Excellent written and verbal communication skills including computer literacy - Demonstrated commitment to working collaboratively with clients, families and other organisations - Demonstrated ability to work in a multi-disciplinary team - Commitment to evidenced based practice, continuous evaluation of service practices and quality improvement - Knowledge of emerging creative and innovative approaches to chronic disease treatment and diabetic education - Current Driver’s licence with excellent driving record - Current Police Clearance and Working with Children Check - Ability to travel throughout the Pilbara region
What's on Offer Time worked outside normal hours will be remunerated through time off in lieu of overtime (TOIL). It is a requirement of this position that you request approval for TOIL.

The Pilbara is located about 1600 kilometres north of Perth, in the north-west region of Western Australia, with an area of around 500,000 square kilometres and a population of approximately 40,000 people. The primary towns are Karratha and Port Hedland on the coast, and Tom Price and Newman inland.

An area of rugged beauty, the Pilbara has a vast coastal plain, spectacular inland ranges and an arid desert region reaching into the centre of Australia. It is characterised by the rich red hues of its landscape, imparted by the abundance of natural riches in its reserves of iron ore. Mining and offshore oil and gas exploration have given rise to the Pilbara’s economic boom in recent years, drawing large numbers of people to the region on both a temporary and permanent basis.

It has a tropical, semi-desert climate with a mainly summer rainfall associated with thunderstorms. From October to April temperatures throughout the region generally exceed 32 degrees celsius every day, sometimes topping 40 degrees, particularly in inland areas.

There are approximately 46 doctors working in the region in various areas, including general practices, Aboriginal Medical Services, hospitals and the Royal Flying Doctor Service.
