Diabetes Educator

About the Role
  • An exciting opportunity is now available for a Diabetes Educator to work on a full or part-time basis, in the Goldfields

    Role Overview 360 Health + Community operates a multidisciplinary health service, treating patients with an identified chronic condition or who have been identified as at risk of developing a chronic condition.

    This role provides diabetes expertise and education to patients to assist general practitioners in the management of those with Diabetes in the region. This includes receiving and attending to patient referrals, the provision of clinical management, care coordination and patient referral to other relevant support services. Services and programs include those that support those with newly diagnosed diabetes and those that support ongoing managements in the primary care setting.

    The employee may also take an active role in coordination, development and facilitation of local initiatives. Such activities should encourage uptake of the available programs and services, enhance community health and drive a process for the integration of local diabetic services across the continuum of care in order to more effectively manage diabetes.
Experience & Skills
  • Qualifications in the area of Diabetes
  • Registration with AHPRA
  • Evidence of working towards credentialing process with ADEA to become a Diabetes Educator
  • Knowledge and experience in the management and prevention of Chronic Disease
  • Demonstrated ability to be able to assess client needs, case manage, deliver services and follow up patients within scope of service delivery and contract funding
  • The ability to deliver in the Goldfields region
  • The ability to maintain accurate electronic records that reflect best practice in service delivery in accordance with 360 Health + Community clinical governance strategies.

The Eastern Goldfields is the largest region in Western Australia. It covers almost one third of the State and has a population of about 54,000. More than half of the region’s residents live in Kalgoorlie-Boulder.

Esperance, on the southern coast, is the second largest centre with a population of around 13,000.

The Kalgoorlie region has been historically significant in the development of Western Australia, as the centre of the gold rush in the 1890s. As well as reminders of those times in the buildings and institutions around the city, there is still a plethora of natural bounty to be enjoyed in its eucalyptus forests, wide sweeping plains and abundance of wildlife. The cultural mix found in the region is also rich, with a melding of Indigenous and European heritages that has created an area of unique character. To the southern edge of the region the pristine beaches provide a stunning backdrop to Australia’s southern ocean, reached through a corridor of National Parks and wide swathes of unspoiled natural beauty.

The primary industries in the cooler southern part of the region are fishing and farming. The warm, dry northern areas are dominated by gold and nickel mining and industry-related services.

About 50 general practitioners work in the region across 22 practices, including Aboriginal Medical Services and the Royal Flying Doctor Service. Kalgoorlie is well serviced by specialists.
